South Metro Human Services is a non-profit agency serving adults with mental illness and/or chemical dependence in the Twin Cities metro area.

We are engaged in activities that range across a spectrum that includes forensic evaluations, therapy, case management, housing, referrals, money and medication management, and other daily living and coping assistance. The staff providing these services come from equally diverse professions, including individuals trained in medicine, social work, and psychology, with various specialties within each discipline.

South Metro Human Services is composed of four divisions: Case Management Services, Residential and Supportive Housing, Homeless Services and Behavioral Health Services.

Transitions
offers a full range of services for individuals who are recovering from drug and alcohol addiction. Our programs provide structure and support to strengthen personal recovery skills in the dimensions of home, health, purpose and community. We foster safe, caring environments where individuals learn and practice new recovery skills and implement changes that support a substance-free lifestyle. Fountain Centers

A Mayo Clinic Health System Program for Substance Abuse and Addiction, has served Southern Minnesota since 1974.  Outpatient services now in 8 locations: Albert Lea, Fairmont, Faribault, Jackson, Mankato, Owatonna, Rochester, Waseca.  Gender specific and adolescent residential services available in the Albert Lea location. A co-occurring track for adult outpatient is offered in our Faribault location and for our adult residential programs in Albert Lea.
