What Happens in the CONSULTING Stage?
People who do not have waiver or targeted case management or a MSHO/MSC+ care coordinator, but need Housing Stabilization Services, can complete the person-centered planning needed through an enrolled housing consultant. Enrolled housing consultants help a person develop a Housing Focused Person-Centered Plan (DHS-7307) and support the person to select their housing transition or sustaining provider.
* Developing a housing focused person-centered plan based on the person’s needs, wants, and goals for housing
* Helping a person make an informed choice in their housing transition or sustaining services provider
* Offering resources related to non-housing goals
* Coordinating with other service providers already working with the person
What Happens in the TRANSITIONING Stage?
Housing Transition Services helps a person plan for, find, and move into housing and may include:
* Helping the person think about preferred housing
* Helping with the housing search and application process
* Developing a budget
* Understanding a lease
What Happens in the SUSTAINING Stage?
Housing Sustaining Services helps a person keep their housing after they have moved in and may include:
* Education on tenant-landlord rights and responsibilities
* Coaching to develop relationships with property managers and neighbors
* Training on how to be a good tenant; lease compliance
* Problem-solving to maintain housing stability
Referral Information
We welcome referrals from waiver Case Managers, Social Workers, and/or Care Coordinators. In cases where an individual lacks a waiver, the referrer or Better Life Housing can arrange for the completion of a Professional Statement of Need (PSN). Additionally, the completion of a Housing Focused Person-Centered Plan (HFPCP) by targeted case managers or care coordinators is necessary. If not completed, Better Life Housing can either offer Consultation services to finalize the HFPCP or provide Transitioning/Sustaining Services. Better Life Housing can also assist referrers in finding an alternative HSS agency for Consultation or Transitioning/Sustaining services.
Services
Housing Stabilization Services are a new state Medical Assistance (MA) benefit for people with disabilities and seniors who are homeless, at risk of becoming homeless, or living in institutions/segregated settings. Housing Stabilization Services are designed to help people with disabilities and seniors find and keep housing. HSS provides housing support through the entire process: to secure housing, to establish long term stability, and prevent eviction.
